The role of Infinite Possibilities (“IP”) in the Math Buddies program is extremely limited; IP only provides (a) a platform to match volunteer tutors with other students in need of tutoring and (b) information and materials to support the program. IP does not screen the volunteer tutors or student, is not responsible for the nature of the connections, behavior of the volunteer tutors or students, and does not design, supervise, control or monitor the tutoring relationships in any way.

Release of liability. In consideration for participation in the IP Math Buddies program (“Program”), I, on behalf of myself and my minor child(ren), binding our heirs, estate, executors, and personal representative(s), hereby fully and forever release, waive, and agree to hold harmless IP and each of its respective directors, officers, agents, representatives, volunteers, and affiliates (hereinafter all referred to collectively or separately as “Released Parties”), from any and all claims, remedies, and complaints of any kind which I and/or my minor child(ren) have or may have, whether known or unknown, arising from or related to participation in the Program. THIS DOCUMENT RELEASES RELEASED PARTIES
FROM LIABILITY TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW,
INCLUDING LIABILITY FOR NEGLIGENCE.

Assumption of risk. I assume all risks of my child’s voluntary participation in the Program, I specifically acknowledge my understanding that the tutoring is provided by uncompensated volunteers who are not educational professionals and are not required to have any specialized training as math tutors.

Severability. The provisions of this agreement are severable.

The undersigned parent and/or legal guardian hereby acknowledges that he/she is aware of IP’s limited role described above and that he/she is at all times responsible for the behavior of his/her child. Further, the undersigned parent and/or legal guardian understands that he/she is responsible for monitoring and supervising his/her own child or the child under his/her care and ensuring their safe participation in the Math Buddies program.

I HAVE READ AND FULLY UNDERSTAND THIS AGREEMENT. I FREELY AND VOLUNTARILY AGREE TO BE BOUND BY ITS TERMS AND CONDITIONS.
